MAKE THE CALL

Initial Call

Call the Division of Developmental Disabilities Call Center at 1-800-361-4491.

Home Tour

Visit a local 310 agency to discuss your loved one’s needs and learn about available services.

Care Assessment

The 310 agency will assist in gathering documentation, including a psychological evaluation and proof of disability before age 18.

Welcome Home!

Once eligibility is confirmed, your loved one will be placed on the waiting list or begin receiving services.
